Executive health program vs. executive physical: what is the difference?

An executive physical is a one-day diagnostic event. An executive health program is a twelve-month engagement that acts on the diagnostics. What each includes, what each leaves out, and when you need both.

Short answer

An executive physical is a one-day battery of tests that tells you where you are, while an executive health program is a twelve-month engagement that retests on a schedule and changes your training, nutrition, sleep, and stress against the results every week, so it changes where you are.

Two different products that get confused

Both are sold to the same man, often by people using the same words. The difference is time.

An executive physical happens once. You spend a day at a clinic. The report is thorough and arrives within a week. Then it sits in your inbox, and the recommendations (“lose 15 pounds, improve your lipids, sleep more”) are yours to execute for the next twelve months with no one watching.

An executive health program happens every day for a year. It starts with the same kind of bloodwork, but the panel is the input to a plan, the plan is adjusted weekly against your wearable and scale data, and the panel is drawn again at month 6 and month 12 to prove the plan worked. Someone notices the day you stop logging.

What each one is good at

The physical is better at breadth of diagnostics. Imaging, cardiac testing, and specialist consults are things a coaching program does not do and should not pretend to.

The program is better at everything that happens after the report. Training that carries over to life, nutrition that survives client dinners and travel, sleep and recovery managed off a wearable, and a retest that shows direction rather than a snapshot.

The honest recommendation

If you have never had a full workup, do the physical. If you have had three physicals in a row that said the same thing and nothing changed, the physical is not the missing piece. If you can afford both, the physical’s findings are exactly what a good program builds on.

Typically a half or full day at a clinic: bloodwork, an ECG, imaging (often a CT or ultrasound), a stress test, vision and hearing, a physician consult, and a written report. It is diagnostic, comprehensive, and finished when you walk out.

Is an executive physical enough on its own?

For a man who will execute the report's recommendations alone for twelve months, yes. Most men do not, which is why the same findings appear on next year's physical. The gap is not information; it is follow-through.

Can an executive health program replace my annual physical?

No. The program is optimization under physician oversight, not primary care. Keep your physician and your annual physical. The program's panels and reports are useful to bring to that appointment.
